How to submit your claim via post?
1. Download and complete relevant claim form(s)
2. Prepare all supporting document(s)
3. Submit completed claim form(s) with all supporting document(s) within 90 days of the hospital discharge date, or of the date relevant medical service is performed and completed
4. Once all required documents are received, your claim will be processed within 7 working days
5. Settlement letter and/or email will be sent to you once your claim is approved

Please keep all original receipt(s) and claim documentation for 6 months from the claim completion date. AXA reserves the right to request additional information/documentation to support each claim.
Basic documents
- Supplement with Tax Residency Self-Certification for Individual/ Non-individual (For Claims)
- Hospitalisation Claim Form I and Disability/Accident/Hospitalisation Claim Form II
- Original hospital receipt(s) and itemized hospital bills
- Original medical expenses receipt(s)
- Copy of ID or passport of the Insured/Policy Owner (if not provided before)
- Copy of Laboratory/X-ray/CT Scan/MRI/Pathology report(s)
- Copy of sick leave certificate
- Copy of Hong Kong public hospital discharge summary (no need to submit Claim Form II in this case)
- Copy of China hospital discharge summary, admission record and front page (no need to submit Claim Form II in the case of hospitalisation in China)
- Copy of Meals breakdown
Additional documents (if applicable)
- Copy of doctor’s referral for specialist or therapeutic treatment (if applicable)
- Copy of Physiotherapy/Occupational therapy report (if applicable)
- Copy of claims settlement advice from other insurer/party (if applicable)
- Proof of claimant's bank account details with the account holder’s name and account number (e.g. copy of bank passbook, copy of debit card/EPS)
- Telegraphic Transfer Request Letter
