Basic documents:

  1. Death Claim Form I
  2. Death Claim Form II (if the Policy has been in effect for less than 2 years from the Policy date or the date of reinstatement, whichever is later)
  3. Copy of ID or passport of the Insured/Policy Owner (If not provided before)
  4. Copy of ID or passport of Beneficiary
  5. Proof of relationship between Insured and Beneficiary (e.g. Marriage Certificate, Birth Certificate)
  6. Original Death Certificate
  7. Original policy document

Additional documents (if applicable):

  1. Original Notarial Death Certificate (if death occurred in Mainland China)
  2. Cancellation of HKID confirmation note from Immigration Department (if death occurred outside Hong Kong)
  3. Proof of household registration cancellation (for Mainland China visitors/Taiwan citizens)
  4. IRS Form W-8 (for non-US entity/trust)/IRS Form W-9 (for US entity/trust)
  5. Supplement with Tax Residency Self-Certification for Non-Individual (For Claims)
  6. Police Report/Traffic Accident Report (Accidental death or unnatural cause of death)
  7. Post Mortem or Coroner's Report (Accidental death or unnatural cause of death)
CLAIMS SUBMISSION
  1. By post: Mail the completed claim form(s) with all supporting document(s) to the mailing address listed on the claim form.
  2. Through Financial consultant assistance: Submit the completed claim form(s) with all supporting document(s) to your financial consultant.
