  1. Completed Outpatient Claim Form(s) with all details filled out
  2. Original payment receipt(s) showing the name of the patient, date of consultation, diagnosis, breakdown of charges, and the official stamp of the service provider(s) or signature of the attending doctor(s)
  3. Copy of referral letter(s)/prescription(s) from a registered doctor (if applicable)
CLAIMS SUBMISSION
  1. Through Emma by AXA app: Fill-in the form(s) and upload the supporting document(s) in Emma by AXA app
  2. By post: Mail the completed claim form(s) with all supporting document(s) to the mailing address listed on the claim form
  3. (For InternationalExclusive only) By email: Send the completed claim form(s) with all supporting document(s) to heathcare@axa.com.hk
