  1. Completed Outpatient Claim Form(s) with all details filled out
  2. Original payment receipt(s) showing the name of the patient, date(s) of consultation, diagnosis, breakdown of charges, and the official stamp of the service provider(s) or signature of the attending doctor(s)
  3. Copy of referral letter(s)/prescription(s) by a registered doctor (if applicable)
CLAIMS SUBMISSION
  1. By post: Mail the completed claim form(s) with all supporting document(s) to the mailing address listed on the claim form
