Please keep the original receipt(s) and other claim documentation for 6 months from the claim completion date. AXA reserves the right to request additional information/documentation to support each claim.

For Hospital and Surgical Claim

  1. The hospital/medical bill(s)/receipt(s) certified by the attending doctor and stating clearly the diagnosis.
  2. Written referral letter by registered doctor for laboratory tests/x-ray/specialist consultation (if applicable)

For Outpatient/Dental Claim

  1. The medical receipt(s) stamped and signed by attending doctor/dentist clearly showing the date of treatment, patient's name and diagnosis/type of dental treatment; the policy number and the name of the Insured must be clearly written on the back of the receipt(s)
  2. Written referral letter by registered doctor for laboratory tests/x-ray (if applicable)
CLAIMS SUBMISSION
  1. On Emma by AXA: Input claim details/ upload claim form(s), and upload all supporting document(s) via the Emma by AXA app
  2. By email: Send the completed claim form(s) with all supporting document(s) to ec.claims@axa.com.hk
