Basic documents:

  1. Completed Hospitalisation and Surgical Claim Form(s) with all details filled out by you and your attending doctor
  2. Original payment receipt(s), including deposit receipt(s).
  3. Copy of statement(s) with breakdown of hospital charges, including daily fees, meals and package charges

Additional documents (if applicable):

  1. Copy of histopathology/endoscopy/diagnostic/laboratory tests reports
  2. Copy of operating theatre summary report(s)
  3. Referral letter(s) from any specialist doctor
CLAIMS SUBMISSION
  1. By post: Mail the completed claim form(s) with all supporting document(s) to the mailing address listed on the claim form.
