Please keep the original receipt(s) and other claim documentation for 6 months from the claim completion date. AXA reserves the right to request additional information/documentation to support each claim.

  1. Completed Outpatient Claim Form(s) with all details filled out
  2. Original payment receipt(s) showing the date(s) of treatment, the patient's name, diagnosis, the official stamp and signature of the attending doctor(s), and the itemized charges for claims over HKD4,000
  3. Document(s) for any visit to a medical practitioner, hospital, clinic, diagnostic center or any other facility where medical services may be provided
CLAIMS SUBMISSION
  1. By post: Mail the completed claim form(s) with all supporting document(s) to the mailing address listed on the claim form
  2. Through Financial consultant: Submit the completed form(s) and supporting document(s) to your financial consultant for assistance
  3. By email*: Send the completed form(s) with all supporting document(s) to acr@aa-international.com.hk
  4. By fax*: Fax the completed form(s) with all supporting document(s) to Global Elite Customer Service Team at (852) 3529 1638
